Abstract

Primary hyperparathyroidism (PHPT) is a common and heterogeneous endocrine disorder associated with skeletal, renal, and systemic complications. Despite the availability of international guidelines, several aspects of its management remain controversial. The aim of this study was to develop a national expert consensus to provide updated and context-adapted clinical recommendations for the diagnosis, treatment, and follow-up of PHPT.

Material and methods: A modified Delphi consensus study was conducted. An interdisciplinary panel of experts evaluated clinical statements organized into ten thematic domains using a 9-point Likert scale.

Results: Strong consensus was achieved for most statements, particularly regarding diagnostic and localization strategies, assessment of skeletal and renal involvement, surgical indications, intraoperative PTH monitoring, and medical treatment in patients not eligible for surgery. Some areas without formal consensus persisted.

Conclusions: This Delphi consensus provides practical and structured recommendations for the comprehensive management of PHPT and highlights areas of uncertainty for future research.
